Skilled Nursing & Rehabilitation

We offer home visits by registered home nurses.

  • Patient found ambulating with a physical therapist, in the home setting.Alert and oriented ×4. Denies shortness of breath. No pain at rest; reports increased pain with ambulation. Ambulates with walker, steady gait.
  • Vital Signs (post-PT): BP 134/54, HR 121, RR 18, SpO₂ 96%. Lung sounds clear bilaterally. Bowel sounds present on auscultation. No BM today
  • Patient receiving IV antibiotics; infusion initiated at 1630 and infused over 1 hour. Tolerated well.
